The Advanced Molecular Diagnostic and Research Facility (AMDF) at ICMR-NICHDR stands as a nationally significant referral center, integrating high-complexity hematopathology, solid tumor diagnostics, cytogenetics, and precision molecular oncology to bridge the gap between advanced genomic research and public health application. As the only government center in the Delhi-NCR region providing comprehensive, free-of-cost molecular diagnostics for referred cases, the facility delivers WHO 2022 defined molecular transcripts for hemtaolymphoid malignancies, including critical risk stratification markers, thereby reducing financial burden and ensuring equitable access to precision medicine for leukemia and other hematological malignancies. A cornerstone of the laboratory's translational mandate is the development of accessible, indigenous technologies designed to democratize molecular testing. The facility has pioneered and filed a patent for a Dried Blood Spot (DBS) based RNA extraction process, a breakthrough innovation that overcomes pre-analytical cold-chain dependencies and stabilizes labile RNA transcripts. This technology facilitates decentralized sample collection from remote and resource-limited settings, effectively expanding the reach of national diagnostic networks and enabling molecular testing in areas previously inaccessible to sophisticated pathology services. Beyond its diagnostic functions, AMDF operates as an active hub for high-impact research into leukemia pathogenesis and prognostic modeling. The laboratory investigates the kinetics of Minimal Residual Disease (MRD) to optimize flow cytometric sensitivity, validates non-invasive biomarkers such as cell-free DNA (cfDNA) concentration ratios for predicting relapse, and explores morpho-molecular correlations in line with the DHR-ICMR mandate for human resource development, the laboratory serves as a premier center for DNB, PhD, and MSc scholars, as well as technical personnel. Through its integrated model of service, innovation, and education, AMDRF contributes significantly to the "Changing Paradigm of AML Care in India," fostering national self-reliance in high-end molecular diagnostics and advancing the frontiers of precision oncology.
